StartDate: 9/22/2025 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY RN Case Manager - Home Health

POSITION DUTIES Home health visits to check on DCP plans; travel will be within a 30 mile radius

MINIMUM REQUIRED QUALIFICATIONS Skilled nursing facility experience as well as Home Health within the last 3 years; CA license

PREFERRED QUALIFICATIONS 5+ years experience

LENGTH OF ASSIGNMENT 13 weeks

SHIFT / HOURS PER WEEK Days, - 8 hours shifts
