A Home Health Agency in Lafayette, IN is currently seeking a Licensed Practical Nurse (LPN) to join their growing team.

About the Opportunity:

• Start Date: ASAP
• Assignment Length: Ongoing (Contract to Hire)
• Schedule: Monday to Friday
• Hours: 8am to 5pm

Responsibilities:

• Provide care for patients in their homes
• Restore and promote the patient back to their optimal health
• Coordinate with physicians and multidisciplinary team members to ensure the patient is receiving quality care.
• Travel to patients' homes

Qualifications:

• Dependable vehicle with current auto insurance under the laws of the state.
• Valid Driver's License
• IN State Licensed Practical Nurse (LPN)
• CPR Certified

Desired Skills:

• Home Health experience
