Position Description
Job Title: Case Manager RN
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Duration: Approximately 4 months
Location: Lake and Marion Counties, FL (Ocala area)
Travel Required: Between multiple skilled nursing facilities
Primary Responsibilities

Primary Responsibilities
- Ensure accurate and complete clinical documentation
- Collaborate with the interdisciplinary care team to support patient care
- Assist providers with care coordination, including therapy and benefit determination (Medicare Part A, B, and D)
- Monitor and respond to changes in patient condition in partnership with the primary Advanced Practice Clinician (APC) and facility staff
- Participate in facility-based meetings, family conferences, and team case presentations
- Support training, tracking, and compliance with quality measures
- Conduct and assist with in-service education for selected audiences
- Facilitate medication reconciliation, nursing rounds, assessments, and documentation
- May transcribe verbal orders from providers in compliance with state RN practice laws and facility protocols
- Manage acute medical situations and chronic condition exacerbations, including diagnostics and interventions
- Support patients during short-term sub-acute rehabilitation episodes
- Active and unrestricted RN license in the state of residence (or ability to obtain by start date)
- Associate degree or higher in Nursing
- Certified in Basic Life Support (BLS)
- Ability to lift up to 30 pounds and perform physical assessments
- Reliable transportation
- Full COVID-19 vaccination (compliance with federal, state, local, and client requirements)
- Ability to pass a pre-employment drug screening (UnitedHealth Group is a drug-free workplace)
- 1+ year of clinical experience (long-term care setting preferred)
- Experience with patients with complex behavioral, social, or functional needs
- Understanding of geriatrics and chronic illness management
- Proficient in electronic and written medical documentation
- Strong communication and time management skills
- Ability to work collaboratively with diverse care teams across multiple settings
- Willingness and ability to travel between facilities
