Salary Range

$65,000.00 - $101,000.00

Overview

The Clinical Triage RN directs patients by a patient centered approach to the appropriate level of care based on the severity of their clinical need. The severity of the patient’s needs are assessed using protocols, algorithms and clinical judgement that can provide patient disposition to urgent care, emergency care, clinic provider appointments, and /or other services. The Clinical Triage RN will support communication to the patient, family and support systems as appropriate. Exhibits optimal partnership with clinical practices, contact center teams and other community services/resources. Facilitates information through appropriate communication and documentation. The Clinical Triage RN role contributes to guiding patients to the most appropriate site of care based upon their stated care needs, and the interruption of frequent acute hospital or emergency department use patterns.

Required Skills And Abilities

  • Demonstrated effective strong organizational, assessment and proven clinical skills among multiple disciplines. Ability to work under the direction of clinicians.
  • Superior interpersonal and communication skills. Works collaboratively and respectfully with other healthcare providers to ensure appropriate on-going care.
  • Ability to interact professionally with the care team, provide exceptional customer service to patients and visitors, and manage patients’ anxiety related to vaccination. Evaluates each session to ensure safe effective care pre and post vaccination.
  • Strong verbal and written communication skills with proven ability to successfully communicate the objectives, practices, and mission of the organization. Strong knowledge of vaccines, composition and how to assist with possible adverse reactions.

Preferred Skills And Abilities

Prior experience in ambulatory case management or care coordination and chronic diseases. Prior experience with EPIC Ambulatory EMR.

Principal Responsibilities

  • Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having knowledge of nursing care standards and principles. 2. Develops, implements, and manages nursing care plans in collaboration with physicians. 3. Interviews patients and records medical history to establish data for treatment plan. 4. Plans and coordinates the admission and discharge of all patients. 5. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 6. Performs patient care procedures by following appropriate policy and procedure. 7. Records patients’ medical histories and symptoms, operates medical machinery, administers treatment and medications, and coordinates patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 8. Identifies education needed to assure competent delivery of patient care for oneself and others in the department. 9. May perform other duties as assigned. Required Education and Experience Graduation from accredited school of nursing.

Health Requirements

This role is a healthcare worker position. Healthcare workers (HCW) are defined as university employees working a healthcare setting who have the potential for direct or indirect exposure to patients, human research subjects or infectious materials including body substance, contaminated medical supplies, devices and equipment, surfaces, or air. HCW have specific health requirements that must be met prior to starting work, including MMR vaccine or immunity, varicella (chickenpox) vaccine or immunity, TB screening, COVID vaccine according to University policy, hepatitis B vaccine or immunity, and annual flu vaccination.
