Are you passionate about mental health and looking to take your nursing career into the field where it counts most? Join our dynamic Assertive Community Treatment (ACT) Team in Mount Vernon, NY, and help change lives—one visit at a time!

What You'll Do

As a vital member of our multidisciplinary ACT Team, you'll provide community-based care for individuals with serious mental illness—right where they live. This is not your typical nursing role; you’ll be on the front lines of mental health care, providing compassionate support and making meaningful connections with your clients.

Your Day-to-day Will Include

  • Administering long-acting injectable medications in community settings.
  • Traveling to client homes/facilities as a team (2-5 visits/day).
  • Documenting progress notes in the electronic health record (EHR).
  • Supporting clients with medication adherence and psycho-pharmacological care.
  • Collaborating with an ACT psychiatrist, RN, social workers, and case managers.
  • Coordinating lab services, pharmacy deliveries, and medical appointments.
  • Educating clients on wellness, nutrition, substance use, and chronic illness.
  • Providing crisis intervention, emotional support, and advocacy.
  • Partner with the RN to deliver high-quality psychiatric care.
  • Administer packaged injections in clients’ homes.
  • Deliver field-based services that truly meet clients where they are.

What You Bring

  • Current NY State LPN License
  • 1+ year of experience supporting individuals with serious mental illness is preferred, but new grads are welcome!
  • Experience with Electronic Health Records (EHR)
  • Ability to travel locally using approved transportation
