SUMMARY OF POSITION:
The Sepsis Nurse Navigator is responsible for facilitating the care of a patient with the diagnosis of sepsis across the healthcare continuum at LMC. Under the guidance of the East Region Director, this position will support the sepsis program by maintaining a strong component of patient/family advocacy with day-to-day review and coordination of multidisciplinary services. The Sepsis nurse navigator will collaborate with the Sepsis Program Coordinator, clinical providers, care coordinators, social workers, and clinic staff, the Community Paramedicine team and Clinical RX-2-Go Pharmacist.

MINIMUM QUALIFICATION(S):
- Bachelor's degree
- Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
- Minimum five (5) years experience
- AHA BLS prior to end of orientation period
- ED or ICU experience
