A healthcare organization in Brooklyn, NY is currently seeking a Registered Nurse to join their Day Program team. This role involves both clinical and case management responsibilities, primarily focused on serving individuals with developmental disabilities.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: 8am to 3:30pm
• Setting: Day Program

Responsibilities:

• Serving as a member of the interdisciplinary team, coordinating health care services
• Arranging medical services and issuing referrals as needed
• Providing follow-up on medical issues and conducting staff training in various health-related areas
• Managing communicable diseases through preventive measures and staff training
• Participating in team meetings, conducting reviews, and maintaining documentation

Qualifications:

• 1+ years of experience as a Registered Nurse in community-based health services
• Graduate of an accredited program of professional nursing
• License and current registration as a Registered Nurse in the State of New York
• CPR certification
• Strong communication, organizational, and interpersonal skills
• Ability to maintain the strictest confidentiality

• Knowledge of adaptive devices and equipment used by persons receiving services
