JOB SUMMARY

Primarily responsible to assess, evaluate and treat pain and palliative care patients in the acute care setting, under the supervision and collaboration of a physician, by using advance knowledge and skills to identify abnormal conditions, diagnose health problems, develop and implement treatment plans, evaluate patient outcomes, and collaborate with or refer to a physician for further care management when appropriate.

Job Duties

Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information. Uses appropriate critical thinking skills to make independent decisions commensurate with the autonomy, authority and responsibility of an Advanced Practice Registered Nurse (APRN). Performs complete, accurate history and physical assessment. In collaboration with other interdisciplinary team members, develops therapeutic plan of care based on assessment, diagnostic test results, patient and/or family wishes/goals to establish diagnosis and therapeutic plan of care. Accurately completes all documentation. Prescribes appropriate treatment, pharmacological agents and/or other non-pharmacological interventions. Responsible for communication with physician and interdisciplinary team regarding variations and evaluation of patient outcomes. Collaborates with physician and other members of the health care team in managing acute and long term medical needs of patients. Provides monitoring and continuity of care between physician visits. Delegates care within scope of practice. Uses advanced knowledge and skills in teaching and guiding patients, their families and/or other health team members. Recommends community resources to meet patient and family needs. Demonstrates flexibility and responsiveness in handling schedule coverage to provide service excellence. Maintains patient confidentiality. Performs other duties consistent with purpose of job as directed.

JOB SPECIFICATIONS AND CORE COMPETENCIES

Education: Graduate of an accredited college of nursing with a Masters Degree with Adult/Geriatric focus.

Licensure: Current Indiana RN license. Possesses or is eligible for Indiana Prescriptive Authority and Controlled Substance Registration. DEA Controlled Substance Registration required. RN/APRN or ELNEC certification preferred.

Experience: Experience in hospice or hospital-based palliative care preferred.

Other Job Requirements

Must secure a collaborative agreement with a population or role-appropriate physician.

Effective interpersonal skills and the ability to develop ongoing relationships with patients and other health team members for the purpose of providing primary care and education

Highly motivated, self-directed professional. Knowledge of professional nursing theory, practice and medical care to give and evaluate patient care. Knowledge of organization policies, regulations and procedures to administer care. Knowledge of common safety hazards and precautions.

Must possess the ability to react calmly and effectively in emergency situations.

Must possess the ability to communicate clearly and effectively and establish/maintain effective working relationships with patients, medical staff and public.

Assigned hours within your shift, starting time, or days of work are subject to change based on departmental and/or organizational needs.
