We are seeking a Part Time LPN or RN to join our Holyoke Program for Assertive Community Treatment (PACT) team, which provides highly integrated outreach and 24/7 residential support to help individuals achieve recovery, wellness, and independence. The PACT model is forensic-focused and designed to reduce arrests, hospitalizations, and incarcerations while promoting community integration.

As a Nurse on the Holyoke PACT team, you will play a critical role in ensuring the physical health and wellness of individuals served while working in a collaborative, community-based setting.

What You’ll Do
As a Nurse on the Holyoke PACT team, you will play a critical role in ensuring the physical health and wellness of individuals served while working in a collaborative, community-based setting.
Key responsibilities include:
- Overseeing the physical health of individuals served, including assessments, referrals, and coordination of medical and psychiatric treatment.
- Managing pharmaceuticals and medical supplies under the direction of the team psychiatrist, in collaboration with other nurses.
- Supporting treatment goals by completing initial assessments, treatment plans, treatment reviews, and documentation.
- Ensuring compliance with MAP regulations for medication administration and documentation.
- Training and guiding staff to ensure MAP policy and procedure adherence.
- Providing safe, timely transportation in your own vehicle for medical appointments, recreational activities, and other needs.

- Associate’s Degree in Nursing or related field (required).
- Current licensure as an LPN or RN in the state of Massachusetts (required).
- Strong interpersonal and communication skills.
- Cultural competence and ability to work with diverse populations, including forensic clients.
- Familiarity with community resources and Motivational Interviewing (preferred).
- Valid driver’s license and reliable, registered vehicle for use during working hours (required).
