Position Description
Duties Description
Duties to include but not limited to:
Conduct/oversee all aspects of patient related investigations as per OMH Policy & Procedure.
Coordinate incident review process
Serves as liaison to the Justice Center and other monitoring agencies. Facilitates positive relations; provides information as requested within required timeframes; collaborates as needed; schedules and coordinates interviews as needed.
Data entry and data management: Review incidents and restraint/seclusion data for trends
Will be responsible for performing a variety of Quality Management and Risk Management functions; to include conducting investigations of significant incidents, patient/family complaints and any events identified by administrative staff.
Interview staff and gather all relevant documentation, complete comprehensive reports on significant incident within 30 days and present investigative reports and relevant incident information to IRC committee members.
Participation in FMEA, Root Cause Analysis; and provide input to leadership based on trends noted through risk reduction activities.
Provide risk management orientation training to new employees and conduct other risk management related trainings.
Participate on various committees such as IRC, EOC, PI, ETC
Complete other Risk Management/Quality Management tasks as assigned.
Must be available for on call work during off-shifts and weekends.

Posted hiring rate includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951), an Increase Hiring Rate ($9,196), Geographic Pay Differential ($35,000), and Down State Adjustment ($4,000). The posted job rate is the sum of the statutory job rate ($85,138), Geographic Pay Differential ($35,000) and the Down State Adjustment ($4,000).
Additional compensation may apply based on shift assignment. 7% shift differential for evening shift. 10% shift differential for night shift.
Some positions may require additional credentials or a background check to verify your identity.

- If you fail to maintain your license and/or registration once you are appointed, action will be taken to remove you from the position.

If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
