Registered Nurse (RN) – ECM/CalAIM

Fully Remote – California

We are seeking a fully remote Registered Nurse (RN) to support our Enhanced Care Management (ECM) / CalAIM programs. These positions will play a key role in improving care for underserved populations, with a focus on pediatrics.

Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Responsibilities:

• Collaborate with field staff to gather patient information and notes.
• Develop and maintain care plans for Medi-Cal populations
• Work closely with the VP of Clinical Operations and team to ensure program success.

Qualifications:

• Active California RN license
• Pediatrics experience
• Knowledge of Medi-Cal and underserved populations
• Ability to work independently in a fully remote environment

Preferred:

• Bilingual (not required, but highly valued)
• Previous experience with CalAIM, ECM, or outreach nursing
• Registered Nurse (RN) – ECM/CalAIM
