Title: Licensed Practical Nurse - ACT Team

Location: Mt Vernon, NY (Onsite)

Employment Type: Full Time, Day Shift, Monday-Friday, 9 AM-5 PM, 35 hours/week

Compensation

Salary: $30.00-33.00 per HR

Description

• Administer long acting injectable medication and support clients with medication adherence.
• Coordinate laboratory services and report results.
• Document services and progress notes in electronic health records.
• Implement and update treatment plans in collaboration with the ACT team.
• Travel to patients' homes and other facilities to deliver direct care.
• Support program RN in managing psycho-pharmacological treatment of ACT clients.
• Provide nursing care in accordance with agency policies and guidelines.
  
  

LPN Responsibilities

• Carry a caseload and provide general case management services.
• Provide a range of treatment, rehabilitation, and support services.
• Make appropriate community referrals for services as required.
• Support RN with developing and leading health education classes.
• Coordinate with pharmacies for timely delivery of medication.
• Participate in interdisciplinary team meetings and staff development trainings.
  
  

LPN Qualifications

• LPN license is required.
• HS Diploma or Associates degree is required.
• 1 year of experience in providing direct services related to treatment and recovery of individuals with serious mental illness.
• Knowledge of electronic health records.
