Registered Nurse (RN) - Behavioral Home Health

Protouch Staffing is seeking a compassionate and skilled Registered Nurse (RN) with experience in behavioral or psychiatric health to join our growing home health team. In this role, you will provide specialized care to patients in the comfort and privacy of their own homes, helping them manage their behavioral and mental health conditions alongside their physical health needs. This is a unique opportunity to provide personalized, one-on-one care and build meaningful relationships with patients and their families.

Responsibilities

• Conduct comprehensive in-home psychiatric and physical assessments of patients.
• Develop, implement, and evaluate individualized, patient-centered nursing care plans in collaboration with the patient, family, physician, and interdisciplinary team (e.g., social workers, therapists, and home health aides).
• Administer and monitor the effects of prescribed psychotropic and other medications, providing patient and caregiver education on proper administration, side effects, and adherence.
• Observe and document changes in the patient's physical and mental condition, including mood, behavior, appetite, and cognitive function.
• Provide therapeutic interventions, counseling, and emotional support to patients and their families.
• Educate patients and caregivers on illness management, coping strategies, and self-care skills to promote independence and well-being.
• Coordinate care with other healthcare professionals to ensure continuity of care and optimal patient outcomes.
• Perform routine nursing tasks, including wound care, vital sign monitoring, and specimen collection, as needed.
• Maintain accurate, timely, and detailed clinical documentation using an electronic health record (EHR) system.
• Act as a patient advocate, ensuring their health and safety needs are met.
• Respond to and manage crisis situations or behavioral emergencies, when necessary.
  
  

Qualifications

• Current and unrestricted Registered Nurse (RN) license in the state of [State, e.g., Texas].
• A minimum of [e.g., one to two] years of recent clinical experience as a Registered Nurse.
• Experience in a psychiatric, behavioral health, or mental health setting is required.
• Home health experience is strongly preferred.
• Strong critical thinking, analytical, and problem-solving skills.
• Excellent communication and interpersonal skills, with the ability to build rapport with diverse patient populations.
• Ability to work independently and manage time effectively.
• Proficiency with electronic health records (EHR) and basic computer skills.
