Now Hiring: Full-Time MDS Coordinator (RN/LPN)

As a vital member of our clinical team, the MDS Coordinator will play a critical role in the accurate and timely completion of resident assessments and care plans. This position is essential for ensuring our residents receive the highest quality of care while meeting all regulatory and reimbursement requirements.

Responsibilities of the MDS Coordinator

  • Monitor and guide the completion of resident assessments in a timely manner, ensuring accuracy and adherence to all state and federal regulations.
  • Involvement and supervision of inputting MDS assessments, meticulously reviewing data to ensure integrity and compliance.
  • Transmit MDS assessments to the state and obtain a receipt of validation, troubleshooting any issues that may arise.
  • Schedule and facilitate care plan conferences on a quarterly and annual basis, actively involving residents, their families, and the interdisciplinary team.
  • Collaborate with the interdisciplinary team to ensure all necessary documentation is completed accurately and on time to support the MDS process.
  • Serve as a resource for staff regarding the MDS process, providing training and guidance as needed.
  • Maintain comprehensive records of all MDS assessments, care plans, and supporting documentation.
  • Stay up-to-date on all changes and updates to MDS regulations and guidelines.
  • Other duties as assigned by the Director of Nursing and Administrator.

Qualifications For The MDS Coordinator

  • LPN OR RN license is required.
  • MDS experience is required.
  • Strong working knowledge of Case-Mix, Federal Medicare PPS process, and Medical reimbursement.
  • Demonstrated ability to work independently or as part of a group, managing a complex workload with minimal supervision.
  • Excellent computer knowledge and proficiency with Electronic Health Records (EHR) systems.
  • Previous long-term care experience is preferred but not required.
  • Outstanding attention to detail and organizational skills.
  • Strong communication and interpersonal skills to effectively collaborate with various team members and resident families.
