Position Description
Job Overview – Psychiatric RN (Case Management):
Compensation: $53/hour
Location: Philadelphia, PA
Schedule: Monday to Thursday, 8:00 AM – 1:00 PM
Our client is hiring a Psychiatric RN (Case Management) in Philadelphia, PA. In this community-based role, you will coordinate care for adults with behavioral health and co-occurring disorders, administer medications and injections, and maintain accurate records in the Credible EMR system. You will also collaborate with case managers and clinical staff while conducting outreach to ensure continuity of care and strong patient engagement.
Responsibilities as the Psychiatric RN (Case Management):
- Medication Management: Administer oral and intramuscular medications, oversee distribution, and monitor adherence to treatment plans.
- Patient Outreach: Follow up with participants who miss appointments to ensure continuity of care and engagement in services.
- Clinical Documentation: Maintain timely and accurate nursing notes, update medication lists, and ensure compliance with Credible EMR standards.
- Care Coordination: Collaborate with psychiatrists, case managers, and community providers to support holistic treatment and wellness.
- Consultation & Education: Provide psychiatric nursing services and educate clients, families, and staff on diagnoses, medications, and wellness practices.
Qualifications for the Psychiatric RN (Case Management):
- Licensure: Active Registered Nurse (RN) license in the state of Pennsylvania required.
- Experience: Minimum of 2 years of psychiatric nursing experience in community mental health settings.
- Technical Skills: Proficiency in EMR documentation, with direct experience using the Credible system strongly preferred.
- Industry Knowledge: Solid understanding of psychiatric nursing care, behavioral health practices, and psychosocial rehabilitation.
- Skills & Attributes: Strong communication, organizational, and analytical skills with the ability to collaborate effectively in a team-based environment.
