Position Description

A leading nonprofit supporting individuals and communities through mental health, social services, and recovery programs is seeking a full timeLicensed Practical Nurse (LPN) who is passionate about making a meaningful impact and helping every person reach their full potential.

Location: Bronx, NY

Schedule: Mon-Fri 09:00am - 05:00pm

Position Type: Full-Time/Perm

Salary: $60,000 plus extra 2k for anyone Bilingual Spanish Speaking

LPN Responsibilities

  • Conduct health assessments and develop care plans within LPN scope.
  • Support clients in accessing medical care, housing, and community resources.
  • Collaborate with RNs and psychiatrists to manage medications and coordinate care.
  • Administer medications, vaccines, PPD tests, and perform phlebotomy.
  • Monitor vital signs, assess medication side effects, and report findings.
  • Maintain accurate documentation in the Electronic Health Record (EHR).
  • Provide culturally competent care and participate in crisis intervention.

LPN Qualifications

  • Licensed Practical Nurse (LPN) in New York State.
  • 1+ years' experience with mental health, substance use, homelessness, or criminal justice populations preferred.
  • New Grads Welcome
  • Knowledge of community resources and support services.
  • Strong organizational, communication, and computer skills.
  • Spanish fluency a plus.
