Position Description
Position Type: Full-Time (7am-3pm OR 11am-7pm) Location: Home-based care with daily school accompaniment
Position Summary: We seek an exceptional therapeutic caregiver and learning facilitator who combines healthcare knowledge with educational expertise to support a remarkable, resilient older child with special needs. This role spans home and school environments, blending caregiving with developmental coaching. Think Anne Sullivan with Helen Keller – we need someone who sees potential, creates breakthroughs, and approaches each day with clinical competence and educational innovation.

About Our Family & Environment: We are two engaged parents who value collaboration while appreciating caregiver autonomy once trust is established. Our accessible home sits on acres adjacent to wetlands, offering opportunities for fresh air activities and nature-based learning. We provide a handicap-accessible van for transportation.
Key Responsibilities:
Home-Based Care & Development: • Provide comprehensive care while promoting growth, learning, and skill development • Design personalized exercises, activities, and interventions with potential for creative autonomy • Create engaging learning opportunities and utilize outdoor spaces for nature-based activities • Maintain meaningful engagement, balancing support with growth expectations
Medical Monitoring & G-Tube Care: • Perform vigilant monitoring with specific measurements 1-4 times daily • Provide g-tube care and feeding (training provided) • Maintain health records daily and recognize and communicate changes in condition • Demonstrate keen observational skills for medical details
School Environment Support (2-3 hours daily): • Escort client to school and provide one-on-one support • Facilitate movement, classroom participation, and social interactions with peers • Promote communication device use throughout activities • Collaborate with teachers and school staff
Equipment & Technology Management: • Manage wheelchair, walker, communication devices, and g-tube supplies • Transport equipment between environments • Maintain and troubleshoot all adaptive equipment
Required Qualifications: • Healthcare background (CNA, Medical Assistant, EMT) OR educational background (Special Education, etc.) • Experience with children with disabilities • G-tube experience OR willingness to learn • Excellent observational skills and medical attention to detail • Comfort in school environments and facilitating peer interactions • Understanding this requires active engagement, not passive care • CPR/First Aid certification (or willingness to obtain) • Ability to transfer 75-pound individual safely • Clear background check • Reliable transportation and valid driver's license (handicap van provided for client transport)
