NOW HIRING: Hospital Nurse Liaison

Responsibilities

• Development, generating and maintaining hospital relationships with CMs/SWs/Physicians
• Assists with patient eligibility for Skilled Nursing and Long-Term Care by performing in-person assessments-when able-reviewing medical records and coordinating with facility admissions department.
• Responds appropriately and completely to incoming referrals in a timely manner.
• Communicate with patients and their families throughout the admission process.
• Interacts with a variety of hospital staff from different departments.
• Coordinates with facility staff to ensure that each patient has a smooth transition through the admission process.
  
  

Qualifications

• Knowledge and experience with marketing strategies, goals and plans
• Knowledge of Basic Computer skills and ability to learn new programs.
• Knowledge of health insurance coverage for patients.
• Must have prior Hospital Care Management or Skilled Nursing Admission experience
• Must have an active LPN or RN compact/multi-state, or MD licensure.
• Must have LSW/CSW Licensure
