Position Description

I. Primary Function/General Purpose Of Position

  • Provide clinical focus on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team.
  • Provides advanced clinical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care.
  • Provides focused assessment to evaluate patients and their support systems for real and potential health and safety issues, educational needs, and resource availability upon initial interaction, throughout care and/or treatment, and at times of transition in collaboration with the multidisciplinary team.
  • Utilizes multiple means of communications including telephone triage, in-person interaction and the use of current and future technologies.
  • Facilitates patient access and progression through the Advanced Cardiac Valve Center
  • Provide education to staff, patients and families as needs are identified.
  • Provides clinical and administrative program support to the surgeon(s), cardiologist (s) and Advanced Practice Clinicians [APC (s)] associated with the Structural Heart program, industry sponsors as well as staff and patients.
  • Maintains communication with referring clinicians
  • Assists the Clinical Research Coordinators [CRC(s)] for Structural Heart related research trials.

II. Employment Qualifications

  • Registered Nurse license in Commonwealth of Virginia, graduate of accredited RN program
  • Bachelor of Science Nursing Degree preferred
  • Current American Heart Association [AHA] BLS Provider certification
  • 3-5 years cardiac surgery and/or cardiology experience as a Registered Nurse

Clinical

III. Essential Job Functions

  • Provide direct patient care through individual patient assessment, diagnosis, management and coordination of care.
  • Coordinate patient care by acting as a liaison between consulting physicians, APCs, nurses and other ancillary services.
  • Organizes and utilizes a team approach to meet the needs of the service line and organization by promoting teamwork to accomplish tasks and goals. Articulates and demonstrates to staff members how to work together to solve common problems and attain shared goals. Facilitates interpersonal, intra/interdepartmental and interfacility relationships. Anticipates customers’ needs and seeks opportunities to help and serve others.
  • Identifies the need for change and process improvement related to cost, quality and service.
  • Maintains accurate records, medication lists, and documentation of care and follow-up for administrative purposes and reimbursement services.
  • Collaborates with members of the interdisciplinary assessment team in the evaluation, planning and delivery of comprehensive assessment with the goal of continuous improvement of quality assessment
  • Demonstrates ability, skill and subject matter expertise in field and acts as a resource to others. Participates on applicable key team activities/projects/committees, and provides expertise in field. Facilitates and evaluates clinical activities directed toward cost-effective, quality patient care.
  • Demonstrates regard for the dignity and respect for all patients, their families, guests, and representatives of other organizations as well as fellow employees, volunteers, and medical staff in support of the practice’s mission to provide consistent, quality health care services in a professional, caring, and responsive environment
  • Demonstrates responsibility for individual performance and efficient utilization of products, supplies, equipment, and time to ensure the timely completion of duties and to promote financial viability through provision of services at reasonable cost
  • Follows established safety precautions and procedures in the performance of all duties in order to ensure a safe environment
  • Identifies, recognizes and reports issues that require administrative, advanced practice clinician or physician attention and/or intervention. Collaborates with administrators, APCs, and physicians to resolve such issues
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served
  • Demonstrates knowledge of the principles of growth and development across the life span
  • Possesses the ability to assess data reflective of the patient’s status
  • Interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs
  • Utilizes critical thinking skills in the formulation and implementation of the patient’s plan of care
  • Potential discharge needs are identified and communicated according to policy
  • Delivers care with respect to the individual’s spiritual and cultural needs
  • Provides patient care in a professional and ethical manner
  • Provides health education to patients about ways to improve, promote and maintain their health status, including but not limited to providing educational information on

disease/disease processes, self-care practices, and positive lifestyle choices (i.e., diabetes, safe sex, weight loss, smoking cessation, etc.)

  • Coordinate additional diagnostic testing and clinical evaluations by Advanced Cardiac Valve Center clinicians
  • Participates in professional development activities

Administrative

  • Attend hospital committee meetings as indicated
  • Ensure TVT documentation compliance
  • Collaborate with institutional financial departments to review billing and reimbursements for structural heart procedures and research patients
  • Maintain open and timely communication with referring clinicians
  • Ensure proper preparation and coordination with industry representatives for each structural heart procedure
  • Coordinate with Transcatheter OR Lead RN as well as Cath Lab Clinical Lead for structural heart procedures
  • Manage transcatheter valve OR schedule
  • Manage Advanced Cardiac Valve Center clinic schedule
  • Maintain database of structural heart referrals to assess timeliness of access to care
  • Assist the Structural Heart Medical Director in their clinical and administrative responsibilities
  • Assist Structural Heart Surgical Director in their clinical and administrative responsibilities
  • Assist the Structural Heart Program Director in their clinical and administrative responsibilities
  • Support the Clinical Research Coordinators for the Bon Secours Heart & Vascular Institute on structural heart research trials

IV. Other Job Functions

Heart & Vascular Institute Research Responsibilities:

  • Coordination of research trial participant diagnostic and procedural care according to trial protocols
  • Coordination of research trial participant follow-up office visits
  • Assist the CRCs with communication with clinical research trial participants referring providers
  • Reporting for research
  • Other duties as assigned

V. Working Conditions

The individual performing the job may anticipate coming into contact with human blood and other potentially infectious materials. Individuals in this position are required to

exercise universal precautions, use personal protective equipment and devices and learn the policies regarding infection control.
