Responsibilities

• Deliver nursing care services tailored to chronic illness, disability, or homebound patients
• Educate and instruct patients and their families on health management and treatment plans
• Provide counselling to patients and families to support achievement of treatment goals
• Establish and consistently follow individualized plans of care for each patient
• Conduct thorough patient assessments, analyses, and develop effective treatment plan
  
  

Requirements

• Valid Driver's License and Auto Insurance
• Home Care experience preferred but not required (new graduate's welcome!)
• Compassionate approach and excellent communication skills
• Valid State RN License
