Position Summary
Conducts chart audits/reviews for clinical practice guidelines, mortality review, blood review, core measures and any other chart audits/reviews necessary.
Reports to: Quality & Patient Safety Officer
Responsibilities of the Position
Employs techniques and tools associated with continuous Performance Improvement to assist teams in achieving goals for improvement.
Works extensively with physicians, pharmacists, nurses, and other hospital staff in achieving support for the practice of evidence-based medicine related to clinical care of sepsis patients.
Develops systems for evaluating outcomes for clinical departments and physician compliance.
Facilities Sepsis-related Teams/task forces/Committees and initiates and coordinates an in-depth analysis on near or actual misses in compliance in collaboration with risk and quality staff and develops strategies to achieve the desired results of measure sets.
Adheres to abstraction rules of CMS, TJC when performing chart abstractions for these entities.
Performs clinical concurrent review, conducts team meetings, and coordinates internal reporting.
Maintains an in-depth knowledge of hospital systems, patient clinical data, EMR capabilities and reports, and external data bases used for public reporting and comparison.
Ensures the integrity of the data, and adheres to abstraction rules of CMS, TJC, DLP Outcomes Measurement, and Ethics and compliance policies.
Responsible for the chart abstraction and data collection for core measures.
May be required to perform other job duties as assigned.
Required Education
Associate's degree in nursing is required.
Bachelor's degree in nursing is preferred.
Licenses/Certification
North Carolina or Compact State active Registered Nursing License is required.
Required Skills
Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.
Minimum Work Experience
Knowledge of content of medical records and chart audits reviews.
Excellent computer skills working with Electronic Medical Records/Documentation systems as well as Word, PowerPoint and Excel for data collection, documentation and presentation purposes.
