Position Description
Registered Nurse – Virtual Care Support
Position Overview:
We are seeking a dedicated Registered Nurse (RN) to support a team of virtual healthcare providers. This role involves guiding patients through their care journey, assisting with clinical tasks, and ensuring smooth coordination between patients, providers, and healthcare services. The RN will work remotely as part of a multidisciplinary team to deliver high-quality care in a telehealth setting.
Key Responsibilities:
- Support patients by helping them navigate the healthcare system and understand their care plans
- Participate in team meetings and contribute to care quality improvements
- Provide health education and support for managing chronic conditions
- Help coordinate referrals and follow-up care with outside providers
- Assist patients after hospital visits or emergency care to ensure proper follow-up
- Manage care for patients with complex medical needs or those at risk for hospital readmission
- Complete preventive health screenings and assist with day-to-day patient needs
- Facilitate clear communication among providers about care plans and updates
- Support medication management following protocols and standing orders
- Help track health outcomes and contribute to data collection and quality audits
- Follow standard procedures for triage, refills, and care escalations
- Use clinical judgment to carry out tasks within the RN scope of practice
- Collect and analyze health data to support patient care programs
- Provide clinical support and guidance to medical assistants and other team members
Requirements:
- Bachelor’s Degree in Nursing (BSN)
- Active RN license in a Compact State (must reside in a compact state)
- Strong verbal communication and interpersonal skills
- Proficient in using computers and digital tools (e.g., Google Workspace)
- Experience working with electronic health records (EHR) or clinical databases
- 2–5 years of nursing experience, preferably in outpatient or chronic care settings
- Willingness to work occasional evenings or weekends as needed
