MDS Coordinator (RN) – Skilled Nursing

Manchester, NH – Skilled Nursing Facility

Full-Time | Monday–Friday | On-Call Rotation

Salary: $75k-$85k/yr + Exceptional Benefits

NorthMed Health is seeking an experienced MDS Coordinator that is also a Registered Nurse to join a highly respected skilled nursing facility in Manchester, NH. About The Facility

Key Responsibilities

  • Oversee and coordinate the RAI/MDS process, including assessments, care planning, triggers, and CAAs
  • Manage PDPM/Medicare & Medicaid assessment schedules and deadlines
  • Lead and support person-centered care planning across interdisciplinary teams
  • Conduct training and audits related to compliance and documentation
  • Maintain Medicare certification records and support reimbursement accuracy
  • Ensure compliance with state, federal, and OBRA regulations

Requirements

  • Active Registered Nurse license in NH
  • RAC-CT certification preferred (or willing to obtain)
  • 2+ years of long-term care experience with RAI process knowledge
  • Strong organizational, documentation, and communication skills
  • Familiar with Microsoft Office Suite (Excel, Word, Outlook, PowerPoint)
  • Ability to multi-task in a dynamic, team-based setting
