Pride Health is currently seeking candidates for a Registered Nurse – Home Health role to support our healthcare partner’s operations in Oxford, AL. Job Description

• Job Title: Registered Nurse – Home Health
• Location: Oxford, AL
• Schedule: Monday – Friday
• Shift: Day Shift (Flexible until shift is complete)
• Pay Range: $50.00 – $54.00/hour DOE

Overview:

We are seeking a compassionate and experienced Home Health RN to deliver skilled nursing care to patients in their own homes across Calhoun and Cleburne Counties. The RN will coordinate care across interdisciplinary teams and maintain high standards of clinical documentation and compliance.

Key Responsibilities:

• Assess patients' physical, psychosocial, and environmental needs in stable and emergent situations.
• Develop and implement individualized Plans of Care (POC) ensuring quality and continuity.
• Deliver care using best practices for infection control and patient safety.
• Educate patients and families on medications, home safety, and self-care techniques.
• Provide clinical supervision to HHAs, STNAs, and LVNs to ensure continuity of care.
• Monitor compliance with third-party payer requirements.
• Participate in on-call responsibilities and respond to urgent patient needs.
• Attend interdisciplinary team meetings and contribute to care planning.
• Document all visits, and care provided in a timely and accurate manner using HCHB software.

Qualifications:

• Active RN license in the state of Alabama – Required
• CPR Certification – Required
• Valid driver’s license, reliable transportation, and auto insurance – Required
• Minimum 1–2 years clinical RN experience – Required
• Minimum 2 years of Home Health experience – Required
• Experience with Homecare Homebase (HCHB) software – Required
• Familiarity with OASIS documentation – Required
• CHPN Certification – Preferred
• BSN – Preferred (associate degree or Diploma in Nursing – Required)
