RN Case Manager – Hybrid (Remote & Field Based)

Daily Responsibilities For RN Case Manager

• Manage high-risk cases supporting children, adults, and seniors with complex medical, behavioral, and social needs
• Develop and implement individualized care plans such as conducting assessments, care coordination, and engaging members using evidence-based approaches
• Conduct field based member visits
  
  

Required Qualifications For RN Case Manager

• Active/Unencumbered RN License in the state of California
• Minimum 3 + years of case management experience in a healthcare delivery setting
• Must have a valid California Drivers License with valid automobile insurance
• Advanced computer proficiency
  
  

Preferred Experience

• Managed care experience!
• Bachelor’s degree in nursing
• Bilingual (English/Spanish)

Pay Details: $91,000 - $121,000 (Dependent on years of experience)

Shift: Monday – Friday | 8:00am – 5:00pm

Location:

• 3 Days Traveling in the Field
• 2 Days Remote – Work from home
