Position Description

Pride Health is currently seeking candidates for RN → Home Health positions that will provide support for our client's healthcare systems in Chapel Hill, North Carolina. This is a contract assignment with a high likelihood of extension and a great way to start working with a top-tier healthcare organization!

Job Description:

Job Title: RN → Home Health

Location: Chapel Hill, North Carolina

Schedule: 40 hours/week

Shift: 5x8 Days (8 hrs)

Contract Length: 13 Weeks

Pay Range for Traveler: $2,000.00- $2,050.00 Weekly DOE

Job Summary:

We are searching for The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring quality, compassionate, and individualized healthcare. The RN is responsible for assessing patient needs, developing and implementing care plans, administering treatments, and educating patients and families on disease management and self-care. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating with an interdisciplinary team to promote patient recovery, independence, and quality of life.

Job Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized home health care plans.
  • Provide skilled nursing care, including wound care, IV therapy, injections, and chronic disease management.
  • Administer and monitor medications, ensuring safe and accurate delivery.
  • Educate patients and families on treatment plans, self-care techniques, and medication management.
  • Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care.
  • Document all patient visits, interventions, and progress in compliance with regulatory and agency standards.
  • Monitor patient condition, report changes, and adjust care plans accordingly.
  • Support patients with post-surgical recovery, rehabilitation, or palliative/end-of-life care.
  • Supervise and guide LPNs, home health aides, and other support staff as needed.

Required Certification & Experience:

  • Experience: Minimum 1 year nursing experience with acute and chronically ill patients.
  • Certification: BLS-AHA.
