You’ll plan, organize, and deliver in-home hospice care, with a focus on patient comfort and family support. Key Responsibilities:

• Conduct comprehensive assessments of patients and families.

• Develop and update individualized care plans, administering medications and treatments as needed.

• Provide nursing care, health education, and emotional support to patients and families.

• Coordinate care with physicians and interdisciplinary teams.

• Participate in on-call duties and ensure seamless care continuity.

Qualifications:

• RN license required and hospice/community health experience preferred.

• Strong communication and case management skills.

• Ability to work independently and compassionately in patient homes.
