Qualifications:

* Current and unencumbered license to practice as a Licensed Practical Nurse in the state of Oregon

* Reliable transportation, auto insurance and willingness to travel to our client's home

* Excellent problem solving and customer service skills. 

Job Duties:

* Ability to collect data, perform focused assessments, document accurately, and communicate finding to the RN

* Explaining procedures and treatment protocols to clients, family members, CNAs an caregivers 

* Technology savvy and connected (daily)

* Understands and follows the Oregon Nurse Practice Act

* Management of incident reports and initial monitoring visits

* Administer medications and/or verify clients are medicating correctly

* Verify that client charts are updated promptly and accurately

* Monitoring client's overall health during every visit

* Changing bandages, wound dressings, and catheters

* On-call rotation for clients who have healthcare emergencies

* Willing ness to travel in company
