Homeland has partnered with a well-established community hospital in North Carolina to hire a Licensed Practical Nurse (LPN) for its Skilled Nursing and Long-Term Care Unit. As a Licensed Practical Nurse (LPN), you’ll be part of a compassionate team dedicated to delivering high-quality care in a warm, supportive environment. You’ll join a dedicated care team within a state-of-the-art 90-bed Skilled Nursing & Rehabilitation Unit, which includes 20 brand-new rehabilitation beds and 70 long-term care (LTC) beds. The unit serves a diverse patient population, including individuals with CHF, COPD, diabetes, renal failure, dementia, stroke, and heart disease.
In this role, the Licensed Practical Nurse (LPN) will care for patients recovering from illness or injury, as well as residents with long-term care needs. You’ll deliver direct patient care under the supervision of an RN or physician, administer medications and treatments, monitor patient conditions, assist with care planning and documentation, support daily living activities with empathy and dignity, and collaborate with a multidisciplinary care team to ensure holistic, coordinated care.
Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse or physician
- Administer prescribed medications and treatments accurately and safely
- Monitor patient conditions and report any changes in a timely manner
- Assist with developing and updating individualized care plans and documentation
- Support patients with daily living activities while promoting dignity and empathy
- Work collaboratively with a multidisciplinary care team to deliver coordinated, holistic care
Requirements
- Active LPN license in North Carolina or a compact state
- Graduate of an accredited nursing program
- Basic Life Support (BLS) certification, or ability to obtain within one week (facility-sponsored)
- Open to new graduate LPNs
- Experience or strong interest in rehabilitation and long-term care patient populations
