Position Description

Job Title: Registered Nurse – Field-Based Home Health Expert | $6,000 Bonus

Company: Home Health Care, Inc.

Location: Southwest Minnesota Region

Schedule: Full-Time or Part-Time (Mon. - Fri. 8:00 AM - 5:00 PM) + Weekend and On-Call Availability

Compensation: $75,000 - $100,000 annually + $6,000 Sign-On Bonus

Key Responsibilities:

Perform skilled nursing visits and in-home assessments
Deliver wound care, medication administration, and health monitoring
Educate patients and caregivers on disease management and home safety
Collaborate with interdisciplinary providers and community partners
Maintain up-to-date records through our electronic documentation system

What You Bring:

Valid RN license in the State of Minnesota
At least 1 year of clinical nursing experience (home care preferred)
Reliable vehicle and driver’s license
Strong independent decision-making skills
Passion for personalized care and meaningful patient relationships
