$35 - 43/hr | Full-time | Flexible shifts + on-call

We’re hiring experienced Registered Nurse (RN) Case Managers to deliver compassionate care in patients’ homes. You’ll assess needs, develop and manage care plans, educate families, and ensure high-quality care that meets state and federal standards.

Requirements:

Current Ohio RN license
1+ year acute care or med-surg experience (home health preferred)
Valid driver’s license & reliable transportation
