Job Title: Integrated Case Manager

Location: Detroit, MI

Duration: 13 weeks of contract with high possibilities of extension.

Guaranteed Hours:40/week

Shift: Days 8a-4:30p, rotating weekends.

Pay rate- $51 - $54/hr.

Principal Duties and Responsibilities:

• Conduct comprehensive assessments of patients and families to evaluate biomedical, psychological, social, and functional needs
• Develop personalized, patient-centered care plans to enhance recovery and care experience
• Engage patients and families through advocacy, education, communication, and resource facilitation
• Apply critical thinking, motivational interviewing, and self-management strategies to help patients overcome care barriers
• Provide counseling for treatment decisions, end-of-life planning, and advance care directives
• Coordinate safe, seamless transitions across care settings
• Advocate for services aligned with patient health plan benefits
• Collaborate with multidisciplinary teams to manage complex medical and social needs
• Lead and participate in collaborative care conferences

Requirements:

• Minimum 5 years of acute inpatient case management experience
• (Telephonic, utilization management, or therapy-only experience does not qualify)
• Epic EMR experience required
• BLS certification through the American Heart Association (AHA)
• Typical case load: 1:21–25 patients (not all require direct meetings — based on risk level)

Education:

• Registered Nurse (RN) with a valid, unrestricted State of Michigan license.
• Nursing degree
