Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST

About The Role

Seeking a compassionate and experienced Acute Transitional Care Nurse (RN) to join our team. This role is pivotal in ensuring seamless transitions of care across inpatient and outpatient settings. You’ll work closely with primary care providers and interdisciplinary teams to optimize patient outcomes, reduce readmissions, and promote continuity of care.

Key Responsibilities

• Coordinate transitions of care using clinical protocols and health plan benefits.
• Engage with patients, families, and caregivers to develop and implement care plans.
• Facilitate discharge planning and follow-up care across various settings.
• Collaborate with physicians, hospitalists, and care teams to ensure quality outcomes.
• Provide remote and occasional in-person clinic support.
• Utilize motivational interviewing and behavioral coaching to support patient self-management.
• Document patient interactions and care plans in electronic health records.
• Travel locally to meet patients and families as needed.
  
  

Required Qualifications

• Bachelor’s degree in Nursing (or Associate’s degree with 4+ years of experience)
• Active, unrestricted RN license in the state of Florida.
• Minimum 2 years of clinical experience in a physician’s office, hospital, or clinical setting.
• Valid Florida driver’s license and reliable transportation.
  
  

Preferred Qualifications

• Experience in care continuum management, utilization review, or discharge planning.
• HMO experience.
• CCM Certification.
