Field Case Manager – Registered Nurse
HealthCare Support is seeking a Field Case Manager – Registered Nurse for a mission-driven organization in Minneapolis that’s dedicated to empowering individuals with complex health needs to live safely and independently in their communities.
Pay Details: $40/hr. – $43/hr.
Shift Details: Monday – Friday, 8:00 AM – 4:00 PM I No weekends or on-call!

Daily Responsibilities For Field Case Manager – Registered Nurse
- Conduct home visits to assess client needs, develop care plans
- Serve as a resource for clients, families, and staff on care coordination, program use, and community services to promote client health and safety
- This role involves no direct patient care, great for someone who is trying to step away from a traditional bedside role!
- Current unencumbered license as a Registered Nurse in the state of Minnesota
- 1+ year of experience as a Registered Nurse
- Prior experience in home health or field-based role
- Valid driver’s license, clean driving record, and reliable transportation to use for work driving purposes
- Medical, Dental, and Vision – Benefits start the 1st day of the month following 30 days of employment
