Job Details

Home Care Field / Visiting RN seeing patients in their homes M-F 5x per week - Also open to 4 days a week. Brooklyn. Seeing multiple patients per day - Pediatrics or Adults (your choice)

What you will be doing

• Complete initial assessments, , recertifications
• Creates and implements a plan of care
• Provides clinical care as needed
• Collaborates with the interdisciplinary team and coordinates patient care
• Educates patients and families
  
  

Skills you need

• RN license in NY, or ability to obtain
• Pediatric acute care experience highly preferred
