Position Title: Acute Transitional Care Nurse (RN)

Department: Comprehensive Clinical Management

Work Location: Telecommuter – Must reside in Treasure Coast, Tampa, Citrus, Northeast Florida, or Orlando area

Work Schedule: Monday to Friday, 8:00 AM – 5:00 PM EST

Essential Functions:

• Prioritizes patient care needs and addresses emerging issues
• Meets with patients, families, and caregivers to discuss care and treatment
• Assists with patient follow-up across care settings
• Uses protocols and clinical resources to optimize outcomes
• Oversees discharge provisions and continuity of care
• Maintains communication with physicians, care teams, and families
• Understands cost consequences of care decisions
• Consults with physicians to implement care plans
• Coaches patients using motivational interviewing
• Coordinates with care team and ensures accountability
• Discusses DPoA and advanced directives
• Provides accurate benefit and resource information
• Documents patient information in electronic platforms
• Collaborates with providers under proper scope of practice
• Provides remote clinic coverage and travels as needed
• Performs additional duties as assigned

Required Qualifications:

• Bachelor’s degree in nursing (or associate degree with 4+ years of experience)
• Active, unrestricted RN license in Florida
• Minimum 2 years clinical experience in physician’s office, clinic, or hospital

Preferred Qualifications:

• Experience in care continuum management, utilization review, or discharge planning
• HMO experience
• CCM Certification

Knowledge, Skills, and Abilities:

• Knowledge of patient care standards and nursing practices
• Valid Florida driver’s license and vehicle for travel
