Title: Case Manager (RN) – Hybrid

About the Role:

We’re looking for a compassionate and experienced Registered Nurse to join our patient support team, guiding individuals through a specialized pain management program. In this role, you will serve as the primary point of contact for patients, providing education, support, and care coordination to ensure they feel confident and informed throughout their treatment journey.

What You’ll Do:

• Conduct introductory calls with patients to orient them to the program and explain the treatment process.
• Educate patients on their therapy, providing clear guidance on what to expect during treatment.
• Serve as a consistent, trusted point of contact, following up regularly to address patient questions and concerns.
• Assist patients in navigating insurance, co-pay, and prior authorization processes.
• Collaborate with internal teams to ensure smooth patient care and program adherence.

Who You Are:

• A Registered Nurse with an active RN license (compact license preferred).
• Experienced in case management, ideally 3+ years in a clinical support or pharmacy benefit management (PBM) environment.
• Comfortable using digital platforms and hybrid work arrangements.
• Skilled in motivational interviewing and patient-centered communication.
• Empathetic, organized, and proactive in providing patient support.

Nice to Have:

• Certified Case Manager (CCM) credential.

Work Schedule:

• Monday–Friday, 40 hours per week. Standard hours typically 8 AM–5 PM or 9 AM–6 PM, with occasional flexibility for patient needs across time zones.

• Remote, hybrid, or in-person options available (onsite presence preferred).
