Overview

Key Responsibilities
● Provide management and delivery of patient care plans
● Support patients in Glen Carbon and surrounding areas
● Reward the accomplishment of key achievements during the first year

Schedule and Shift Details
●: 7 days on followed by 7 days off: Mon-Thurs: 5P to 8A; Friday- Sunday: 8A to 8A (would more the less be a on call position during the weekends) - (2) holidays per year – 1 summer & 1 winter; 3 floating holidays

Required Qualifications
• Associate degree or higher
• 2+ years of experience with Hospice
• 1+ years of experience with TruChart EHR or Other EMR or Electronic Medical Record (EMR)
• Valid Registered Nurse (RN) certification/license in Missouri (MO)
• Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)?
• Have you spoken with a Relode Recruiter about this role (via phone or video)?
• Are you okay with the required travel for this position?
• Have you applied to or worked for Residential anytime in the last 6 months?

Preferred Qualifications
• Please provide your RN License number. (Required for Submittal)
