The Registered Nurse (RN) at Triple C Housing plays a vital role in supporting five individuals residing in a group home with mental health needs through comprehensive health assessments, care planning, and wellness monitoring. This position involves collaborating with a multidisciplinary team to develop individualized recovery plans, ensuring compliance with state regulations, and promoting health education and maintenance. The RN is expected to uphold the agency's mission and values by demonstrating excellence, collaboration, respect, and accountability in all aspects of care.   

Responsibilities:

Clinical & Compliance:

  • Conduct intake, reassessment, and discharge nursing evaluations.
  • Participate in the development and approval of Individual Recovery Plans.
  • Ensure compliance with DMHAS, CMS, and agency policies.
  • Perform face-to-face health monitoring every 60–90 days.
  • Stay current with regulatory changes and participate in audits and inspections.
  • Supervise medication management and chart documentation protocols.

Service Delivery:

  • Provide health education to consumers, families, and staff.
  • Evaluate and respond to immediate medical needs and recommend interventions.
  • Collaborate with treatment teams and participate in case reviews.
  • Review and advise on medication regimens in coordination with prescribers.
  • Support wellness and recovery through evidence-based practices.
  • Maintain relationships with healthcare providers and community partners.
  • Participate in on-call rotation as needed.

Administrative:

  • Document care plans, progress notes, and billing information accurately and timely.
  • Prepare reports and contribute to quality improvement initiatives.
  • Supervise and support clinical staff as assigned.
  • Complete required training and additional assignments as directed.
