II.     Job Summary:

 

This full-time position will provide skilled nursing and health care services to Home Care Services residents living with HIV/AIDS. This position will also follow compliance rules with medically related local, state, and federal regulations that pertain to the programs and services of Clare Housing. This position requires maintaining professional boundaries with Home Care residents who live with physical illness, mental illness, and addiction. Clare Housing is an agency committed to addressing social inequities. This includes addressing injustices for lesbian, gay, bi, and transgender persons as well as a strong commitment to racial justice and racial equity work.

 

The Lead Registered Nurse will authentically implement harm-reduction and housing first strategies into their nursing with Home Care residents. 

 

Annually $63,200.00-$83,400.00 Regular Weekday Hours: 8AM – 4PM. III.    Core Job Functions:

 

Duties and Responsibilities:

  • Provide nursing consultation, assessment, and triage for Home Care Services (HCS) residents’ health needs.
  • Complete nursing assessment of all new Home Care Services candidates; implement service agreement with residents; Assure HCS residents meet the admission and continuing stay criteria.
  • Coordinate all medical related needs of new HCS resident’s – pharmacy, clinics, readiness of client apartment – with Supportive Service Managers (SSM) and Project for Pride in Living (PPL).
  • Develop care plans for each individual HCS resident’s needs and health care provider orders and revise, modify, and adapt as needed.
  • Be knowledgeable of compliance issues of the Home Care Comprehensive license and with medically related local, state, and federal regulations pertaining to programs/services.
  • Provide daily direction and every 90-day assessment/observation of Resident Assistants performance of delegated nursing tasks. 
  • Perform nursing assessment of HCS residents within 14 days of admission and at least every 90 days thereafter.
  • Monitor and maintain resident paper charts and charting of medications on EHR program; and write clinical progress notes weekly and as needed.
  • Communicate to health care providers and county caseworker’s changes in HCS resident’s needs or conditions.
  • Assure follow-up on residents’ medical visits, lab, or x-ray services, dental, or eye appointments.
  • Fill weekly pillboxes for medical administration by Resident Assistant (RAs are unlicensed staff).
  • Delegate medical administration and nursing tasks to RA’s and Assure medication administration procedures are followed properly and implement staff corrections if needed.
  • Review each HCS resident’s eMAR weekly to ensure documentation accuracy.
  • Provide orientation training to all Resident Assistants on medication administration, HCS policy and procedures, delegated tasks, OSHA and universal precautions, document training.
  • Work as part of an interdisciplinary team to coordinate care for HCS residents with fellow Clare Housing staff and other service providers
  • Scheduling and attending medical appointments with Home Care residents
  • Communicate client changes to other Clare Nurses especially concerns needing the attention of the on-call nurse.
  • Responsible for ordering medications, medication refills, as well as any follow-ups with physicians, including monitoring of and communications regarding PRN use.
  • Responsible for tracking and ordering of medical equipment, medical supplies, and PPE
  • Assure that staff clearly understands and follows all procedures.
  • Assist in the development of policies, procedures, and implementation.
  • Review annually and provide on an as needed basis, names, reason taken, and side effects of medications taken by HCS residents.
  • Be proactive in communications with Team leads and Resident Assistants.
    • Triage all phone calls from RA staff on evening and weekends and provide nursing assessment based on information received by staff and/or direct communication with HCS client.
    • Provide direction to RA staff on delegated nursing tasks and other duties as needed
    • Follow healthcare providers’ orders, coordinate with hospital discharge staff, Geritom pharmacy and adapt treatment plans as needed for HCS resident.
    • Document in EHR system all telephone conversations and assessment with HCS resident and staff in a timely manner.
    • Be knowledgeable of compliance issues of the Home Care Comprehensive license and with medically related local, state, and federal regulations pertaining to programs/services.
    • Perform post-hospital discharge assessment on site for in-patient hospital stays and ER visits
    • Attend all mandatory RN meetings, staff meetings and client care conferences as needed.
    • Be familiar with and adapt as necessary HCS residents care plans.
    • Communicate directly with Social Service Manager any changes in client condition or other concerning matters on a weekly and/or daily basis. 

 

 

IV.    Position Requirements:

 

Education and Experience:

  • 2 years of work experience as a Home Care and/or Registered Public Health Nurse.

 

Knowledge, Skills, and Abilities:

  • Knowledgeable and experienced with harm-reduction, trauma-informed, and housing-first models of care.
  • Have a sensitivity and aptitude for working with individuals who were formerly unhoused and persons living with HIV/AIDS, mental illness, and addiction. 
  • Knowledgeable of HIV/AIDS disease and medications.
  • Ability to work respectfully and communicate effectively with multi-cultural, diverse populations in what can be at times stressful and emotional situations.
  • Ability to read, write and speak English sufficiently in order to communicate pertinent medical and social services to residents and to communicate with management and staff.
  • Ability to relate professionally with residents, staff, healthcare providers, and case managers.
  • Ability to work with an interdisciplinary team.
  • Ability to work independently, exercise good judgment and pay close attention to detail.
  • Excellent interpersonal communication skills; verbal and written.
  • Ability to maintain confidentiality of client information.
  • Ability to maintain professional boundaries and practice self-care in a position that is confronted often with vicarious trauma related to physical illness, mental illness, and substance use.
  • Ability to instruct/supervise staff on medication administration and procedures.
  • Knowledge of local, state, and federal regulations associated with Home Care Comprehensive programs.

Licenses and/or Certifications

 

Required:

  • Registered Nurse currently licensed with the State of Minnesota.
  • CPR/First Aid Certification required; training provided to obtain certification.
  • Driver’s License, State ID or Passport. 

 

Preferred:

  • ACRN (HIV/AIDS Certified Registered Nurse)

 

V.     Working Conditions and Environment:

 

This position is located in a supportive housing building for adults living with HIV/AIDS and other disabilities. Many of the residents are formerly unhoused individuals. The building is accessible and smoke free. This position works during normal business hours which may include evening and weekend hours. The person in this position will be exposed to body fluids, infections, odors.
