Inpatient Case Manager, RN or LVN, Needed in Mission Hills, CA

$37–$50/hour | Full-time | Monday–Friday | On-site | Sign-on bonus offered!

Our client, an innovator and leader in the managed care industry is looking for a compassionate Inpatient Case Manager to coordinate care for hospitalized patients, ensuring they receive the right level of care, smooth discharge transitions, and continued support across the care continuum.  Additional training in Case Management is available!

What You’ll Do
• Oversee inpatient care from admission through discharge
• Collaborate with physicians, nurses, and social services to create effective care plans
• Coordinate post-discharge follow-ups and ensure continuity of care
• Support quality improvement and patient satisfaction initiatives
• Serve as a vital liaison among patients, families, and the care team

What You’ll Bring
• Current California RN or LVN license
• Acute care experience required, Case Management experience preferred
• Familiarity with Discharge Planning or Utilization Management is preferred
• Strong communication, organization, and documentation abilities
• Proficient in Microsoft Office
