Adventist Health is seeking a Registered Nurse (RN) Home Health PCU - Progressive Care Unit for a nursing job in Portland, Oregon.

This position is located in office, Monday through Friday, 8am to 5pm with a weekend rotation.

Job Summary:

Receives electronic faxes, scans documents, and uploads files into the electronic medical records system, Homecare Homebase. Works on routine assignments within defined parameters, established guidelines and precedents. Maintains agency medical records in a timely and accurate manner to ensure documentation flows from referral to final billing in an efficient and effective manner. Maintains current patient information and enters activity into Homecare Homebase information system and processes and tracks medical records requiring manual and scanning processes.

Job Requirements:

Education and Work Experience:

  • High School Education/GED or equivalent: Required
  • One year working in a medical office environment: Preferred
  • Experience with Homecare Homebase: Preferred

Essential Functions:

  • Maintains the medical records of the agency. Participates in agency back office daily operations as assigned by the Office Manager. Retrieves, sorts, scans, and assembles any paper or electronic medical records into proper chart order. Ensures that records are continuously updated, and that required documents are provided, signed, and dated according to agency procedures.
  • Calls hospitals and facilities to locate and confirm information, including but not limited to: patient, hospital, hospital reason, admission date, discharge date, room number and additional notes as applicable.
  • Answers and evaluates needs and importance of calls related to patients and potential referrals. Assists clinicians with appropriate disposition of calls to the agency. Logs all collected patient information and emails to clinicians and office staff.
  • Reviews reports in Homecare Homebase and faxes to physicians to obtain signatures as needed. Tracks missing/late documentation. Files incoming documents daily. Maintains a control procedure for active patient files. Maintains a tracking system to ensure clinical records are current according to policies, procedures, and regulatory and accreditation requirements.
  • Utilizes Homecare Homebase for workflow processing of records received, sent for signature, returned to the agency, and scanned.
  • Performs other job-related duties as assigned.
