Position Description
Job Summary:
LPN focuses on the skilled nursing aspects of care within the adult day health setting, ensuring all care provided is safe, effective, and fully compliant with regulatory standard. The LPN role is regulated by DCH/DBHDD protocols, that involves working as part of a healthcare team (under RN oversight) to provide direct nursing care, monitor client health, and ensure compliance with established policies. Ideal candidates must have a proven track record of competency and compassionate care, with strong leadership and communication skills.
Job Qualifications:
- A valid GA LPN license is required.
- Experience working with the special needs adults or elderly population, preferred.
- Strong communication skills are necessary for interacting with participants, families, and the healthcare team.
- Attention to detail and critical thinking skills are essential for monitoring health changes and making sound judgments.
- Experience or certification in basic life support, such as CPR, required.
Core responsibilities:

Patient care:
- Administering medications and monitoring their effects.
- Taking and recording vital signs and other patient data.
- Assisting with daily activities such as bathing, dressing, and eating.
- Providing basic wound care, like changing bandages.
- Responding to acute changes in a participant's health.

Health management and documentation:
- Conducting health assessments and contributing to the development of care plans.
- Documenting patient care, updates, and observations in health records.
- Reviewing and implementing physician's orders.

Communication and coordination:
- Communicating with registered nurses, physicians, and other healthcare providers about patient status.
- Coordinating with external service providers.
- Educating participants and their families about health conditions and treatments.
- Acting as an advocate for participants.

Staff support and training:
- Training other staff members on universal precautions, infection control, and medication administration.
- Supervising other personnel, such as program aides, in providing care.
