We’re seeking a compassionate Registered Nurse (RN) to join our team and provide exceptional care to patients and families during their hospice journey in the vibrant city of Toledo, OH.

What You’ll Do

Provide in-home nursing care & symptom management
Develop and oversee individualized care plans
Educate & support patients and caregivers
Collaborate with healthcare providers & care teams
Maintain accurate EMR documentation & ensure compliance

What We’re Looking For

Active RN license in Ohio
1 year of acute care or med-surg experience (hospice preferred)
Strong communication & assessment skills
Valid driver’s license & reliable transportation
