What You’ll Be Doing

• Administer long-acting injectable medications and help clients stay on track with treatment.
• Coordinate lab services and follow up with test results.
• Travel to homes and community facilities to deliver direct care.
• Assist the RN with managing psycho-pharmacological treatment.
• Document all services and progress notes in electronic health records (EHR).
• Help develop and update treatment plans alongside your ACT teammates.
• Provide case management, community referrals, and daily support services.
• Support health education efforts and medication coordination with pharmacies.
• Participate in team meetings and continuous staff development.
  
  

What You’ll Need

• NY LPN license.
• 1+ years of experience working in Mental Health or Behavioral Health.
• Familiarity with EHR systems.
• Strong communication and teamwork skills.
• A commitment to recovery-oriented, community-based care.
