Registered Nurse – Home Care

They are seeking 3 Registered Nurses (RNs) to join their Home Care team in Nashua, Greater Londonderry, and Greater Merrimack.

Key Responsibilities

• Perform initial and ongoing home health assessments for patients and families.
• Utilize the nursing process to plan, deliver, and evaluate individualized care.
• Complete comprehensive OASIS assessments and documentation.
• Develop and implement personalized care plans with input from patients and families.
• Serve as the primary case manager, helping patients heal and regain strength and independence.

✅ Qualifications

Required:

• Graduate of an accredited nursing school (ADN or higher).
• Minimum 1 year of recent acute care experience (Med/Surg or Acute Rehab preferred).
• CPR certification.
• Clean background and driving record (mileage reimbursement provided).

Preferred:

• BSN degree.
• Prior Home Health or Community Health experience.
• Familiarity with OASIS documentation and EMR systems.
• Additional certifications (e.g., IV, Ostomy, Chemotherapy).
