Position Description

Location: This position is based in Salem, Oregon, and covers the Salem area only. Portland is not included in the coverage area.

The Primary Care Nurse is an integral part of Housecall Providers’ interdisciplinary team-based model of care. The nurse provides a combination of in-home, community, and telephonic support to patients with multiple chronic conditions, advanced illness, and other complex medical and psychosocial needs. The nurse works with the team, outside organizations, and families to provide patient-driven care. The nurse provides care to identified primary care patients who will benefit from increased support to improve quality of life, manage and relieve symptoms, and improve psycho-social needs.

Estimated Hiring Range

$53.69 - $65.62

Bonus Target

Bonus - SIP Target, 5% Annual

Essential Responsibilities

  • Make initial and on-going visits to assess patients and provide baseline and comprehensive data.
  • Provide hands on skilled care that sometimes includes urinary catheterization, blood draws, injections and basic wound care
  • Utilize nursing process to provide patient care that is individualized, goal directed and consistent with current standards of practice.
  • Identify and communicate the needs of the patient and family to the interdisciplinary group which includes the PCP, assists with identifying goals of care, and monitors and documents progression toward goals.
  • Participate as a member of the interdisciplinary group to establish and implement a biopsychosocial care plan in patients with serious, complex and/or chronic illness.
  • Educate patients and families regarding disease process, pain and symptom management, and what to expect in the event of disease progression. Utilize skills and resources to ensure that education is at the correct education level.
  • Counsel patients and families regarding end-of-life issues and provide guidance throughout the trajectory of illness.
  • Maintain high standards of professional practice by adhering to the ethics of the profession which includes seeking continuing education to support a goal of excellence in nursing care.
  • Relate to patients and families in an open, empathetic and supportive manner while maintaining personal and professional boundaries.
  • Provide hospitalized patient coordination, conduct urgent nursing visits, and obtain lab specimens as requested by PCPs.
  • Work closely with PCPs, Advanced Illness Care and Hospice team to ensure a continuum of care.
  • Complete timely documentation and regularly updates care plans.
  • Responsible for documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards per policies and procedures.
  • Accept and perform other duties as assigned.
  • Maintain patient/family and staff confidentiality in compliance with HIPAA laws.
  • Participate in the coordination of the patient’s care in accordance with professional standards of practice.
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.
  • Develop and maintain professional boundaries with all patients/family/caregivers.
  • Maintain agreed upon work schedule and meet deadlines for completion of work.
  • Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing facilities, assisted living environment, etc.
  • Serve as an ambassador for CareOregon and Housecall Providers at all times.
  • Demonstrates cooperation and teamwork and maintains collaborative relationships with all levels of staff and community partners.
  • Participate in orientation of new employees, volunteers or students seeking experience in primary care
  • Attend staff and mandatory in-services meetings, as required.
  • Participate in ongoing education and assume responsibility for updating professional skills
  • Familiar and comfortable with a compassionate, person-centered approach to communication, education and care planning.
  • Maintain patient and personal safety regarding occupational health and safety and participates in safety program, as requested.
  • Participate in Quality Assessment Performance Improvement efforts to continually assess and improve quality of care, as requested.

Required

Experience and/or Education

  • Current unencumbered license as a registered nurse in the State of Oregon
  • Minimum 2 years’ experience working as an RN in one or more the following areas: Home Health, Hospice, Palliative Care, Community Health, Hospital Care Management, Behavioral Health/Chemical Dependency, or Primary Care/Specialty Care clinics
  • CPR certification at hire or within 6 months in position
  • Valid driver’s license, acceptable driving record and automobile liability coverage or access to an insured vehicle

Preferred

  • Bachelor of Nursing degree
  • Experience working with a diverse community of individuals whose health has been impacted by social determinants, trauma, and marginalization
  • Experience utilizing motivational interviewing techniques, and trauma informed interdisciplinary approach to care

Knowledge, Skills And Abilities Required

Knowledge

  • Knowledge of impact of generational poverty and other social determinants of health on clients’ ability to develop and follow self-care goals to improve health outcomes
  • Familiar and comfortable with a person-centered approach to communication, education, and care planning
  • Comfortable supporting patients at the end of their life; including facilitating, evaluating, and coaching advanced care planning conversations, often in collaboration with other members of the interdisciplinary team
  • Knowledge of Federal, State and Local regulations, HIPAA privacy rules and regulations
  • Knowledge of community resources to support a complex, vulnerable population

Skills And Abilities

  • Demonstrates ability in ongoing case management as well as urgent assessment and triage
  • Ability to exercise sound clinical judgment, independent analysis, critical thinking skills and knowledge of medical and behavioral health conditions when identifying clients’ multidisciplinary needs, developing health goals, and communicating with interdisciplinary team members.
  • Computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet
  • Excellent interpersonal skills
  • Ability to engage with a member, family or caregivers in various states of pain, trauma, and tragedy
  • Excellent communication skills including verbal and written communication in the form of presentations, meetings, emails, reports, networking, and conversation

  • Ability to operate a motor vehicle
