POSITION SUMMARY

The Advanced GI Nurse Coordinator works in partnership with the GI providers and interdisciplinary team to promote quality of care through collaboration with all team members, patients, families, and significant support personnel to help facilitate patient throughput and appropriate discharge planning.
Primary Accountabilities
- Serves as a liaison to GI Hospitalists, Advanced Practice Provider (APPs), staff, patients, and their families to facilitate safe and effective flow of patient care.
- Collaborates with the GI Hospitalist providers to facilitate patient’s transitional plan in partnership with the attending, nursing, and interdisciplinary team.
- Rounds on discharge patients prior to leaving to enhance patient experience, closing the loop on any GI related inquires the patient or family may have.
- Facilitates and promotes high quality, cost-effective outcomes, focused on the whole patient orientation and self-management decision support, minimizing any fragmentation of healthcare delivery.
- Provides service, education and resources to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences, responding appropriately to unexpected, stressful situations while professionally interacting with all, including co-workers.
- Delivers appropriate delegation and supervision of patient care activities in conjunction with Manager; monitor patient outcomes to meet or exceed regulatory compliance.
- Monitors and ensures accurate, timely records from admission to discharge, while working collaboratively throughout the systems to access and update information, and to support the delivery of quality service.
- Manages patient and family inquiries, facilitating the completion of necessary paperwork and documentation, primarily after discharge.
- Coordinates and simplifies post discharge communication with Primary Care Providers (PCPs) and patients, as necessary.
- Follows-up on outstanding diagnostic test results, pending consultations, patient requests or barriers for discharge.
- Models accountability and other expected behaviors in all aspects of job performance for self, the team and department success, including flexing workload and schedule to serve patients as needed.
- Supplies patient, family, and caregiver education as directed by the plan of care; promoting healthy behaviors as much as possible.
MINIMUM QUALIFICATIONS
- Education: Associate’s degree in Nursing (ASN) or Nursing Diploma.
- Work Experience: One (1) year of experience in a GI Acute care setting.
- Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
- Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.
- Skills/Knowledge/Abilities:
- Proven relationships with physicians.
- Strong detail-orientation and follow-up capacity.
- Excellent organizational and critical thinking skills
- Demonstrates success in relationship building
- Ability to keep team focused to meet goals and timeliness
- Ability to work autonomously with minimal supervision
- Intermediate computer skills e.g. navigation and workflow within Epic, medical management applications and Microsoft Office applications.
- Ability to maintain composure in stressful hospital environment
- Education: Bachelor’s degree in Nursing (BSN).
- Majority of time involves sitting or standing; occasional walking, bending, and stooping.
- Long periods of computer time or at workstation.
- Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
- May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
- Communicating with others to exchange information.
- Visual acuity and hand-eye coordination to perform tasks.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
