Now Hiring: Hospice Registered Nurse (Hospice RN)

Location: VNA Hospice of the Southwest Region – Rutland, VT

Contract Length: 12 Weeks

Shift: Monday–Friday, 8AM–4PM (Some weekends/on-call required)

Guaranteed Hours: 37/week

Pay Rate: $70/hr | OT: $80/hr | Weekly: $2,590

Key Responsibilities:

• Conduct comprehensive patient/caregiver assessments
• Develop and implement personalized care plans
• Serve as Case Manager for hospice patients
• Participate in IDT meetings and interdisciplinary planning
• Deliver advanced symptom and pain management
• Delegate tasks per LPN scope and provide clinical supervision
• Educate patients and families on care plans, medications, and hospice services
• Coordinate with providers, facilities, vendors, and insurance for effective patient care
• Ensure timely documentation and compliance with State/Federal and agency regulations

✅ Requirements:

• Active RN license (Vermont or compact)
• Prior hospice or palliative care experience preferred
• Strong documentation and communication skills
• Proficiency with high-tech skills (IV, wound care, VAC, etc.)
• Comfortable with weekend/on-call rotation
