A leading nonprofit supporting individuals and communities through mental health, social services, and recovery programs is seeking a full time Licensed Practical Nurse (LPN) for their ACT team. Compensation: $60,000 Per Year + 2k for anyone Bilingual in Spanish

Key Responsibilities

Conduct health assessments and develop care plans within LPN scope.

Support clients in accessing medical care, housing, and community resources.

Collaborate with RNs and psychiatrists to manage medications and coordinate care.

Administer medications, vaccines, and perform phlebotomy.

Monitor vital signs and report findings.

Maintain accurate documentation in the Electronic Health Record (EHR).

Provide culturally competent care and participate in crisis intervention.

Qualifications

Licensed Practical Nurse (LPN) in New York State.

1 year experience with mental health, substance use, homelessness, or criminal justice populations preferred.

New Grads Welcome.

Knowledge of community resources and support services.

Strong organizational, communication, and computer skills.

Spanish fluency a plus.
